I have suffered from Cerebellar Ataxia since 2004. This is an incurable, long-term, progressive, neurological condition which affects mobility, balance and coordination. I also have diet-controlled Diabetes Type 2.

I submitted my PIP Claim form (PIP.1003) on 03.05.2017, followed by medical evidence on 17.05.2017. I submitted further evidence and additional information on 12.06.2017.

My medical assessment took place at home on 19.07.2017. This was conducted by a Health Professional* working for Independent Assessment Services (IAS, formerly Atos Healthcare) and involved a question and answer session about daily and mobility activities. The assessment was recorded by prior agreement.
